Perimenopause has over 100 documented symptoms. Here are the ones I see most.
If more than three sound familiar, perimenopause is likely part of what’s happening. That doesn’t mean you need HRT — it means you need someone to look properly.

Section one
Physical symptoms
- Hot flashes and night sweats
- Sleep disruption — especially early-morning waking with your heart pounding
- Weight gain (midsection) that won’t respond to diet or exercise
- Joint pain — knees, hips, wrists, fingers
- Frozen shoulder (yes, really — it’s hormonal)
- Heart palpitations, especially at night
- Migraines or new headaches tied to your cycle
- Vertigo, dizziness, tingling skin
- Skin changes, hair thinning, new chin hairs
- Vaginal dryness, painful intercourse, recurrent UTIs
- A different body odor than you’ve had your whole life
Section two
Mental & cognitive
- Brain fog — forgetting words mid-sentence, losing your train of thought
- Difficulty concentrating on tasks you used to breeze through
- Anxiety — often new, often disproportionate, worst in the 2–3 days before your period
- Low mood and irritability (sometimes misdiagnosed as depression or bipolar II)
- Loss of motivation and of the drive you used to have
- Rage — a specific, volcanic anger that surprises you


Section three
Cycle changes
- Shorter or longer cycles, missed cycles, or doubled-up cycles in the same month
- Much heavier bleeding or “flooding” — sometimes with clots
- Spotting between cycles
- PMS that is suddenly much worse than it used to be
